Rebuilding a patient portal used by 400,000 people without a single day of downtime

Meridian Health Network's patient portal had grown into a maze of unsupported PHP code that no longer met modern compliance or accessibility requirements, but it was also the primary way 400,000 patients booked appointments, viewed lab results, and messaged their care teams. We ran the rebuild as a parallel system behind a routing layer, migrating patient cohorts in phases rather than attempting a single cutover. Every phase was validated against the live system before the next group moved over, and the legacy portal stayed available as a fallback until the migration was fully verified. The result was a modern, WCAG 2.1 AA-compliant portal built on Laravel and a HIPAA-aligned infrastructure, delivered without a single appointment missed or a support ticket caused by downtime.
